Southwestern Eye Center is providing notice of a potential privacy incident, detailed below, that we recently identified. We have notified the individuals affected by the incident via first class mail at the patient’s last known address in accordance with Federal regulations.
We learned that refund checks mailed to individuals on April 17, were made out to the wrong individual as payee. We conducted a thorough investigation and determined that an administrative processing error by one of our vendors caused the checks to be made out to the wrong individual. While the refund checks contained the wrong individual’s name, they did not expose any other personal, health, financial or billing information.
At this time, Southwestern Eye Center has no reason to believe that individuals are at risk of identity theft or that they should take any particular measures to protect themselves from harm as a result of this incident. We have requested that recipients shred or destroy the refund check received in error.

We take our role of safeguarding patients’ personal information very seriously. Accordingly, we have implemented measures to prevent this type of incident from occurring again, including providing training and additional quality control in our mailing processes and oversight over vendors.
