We want your time in our Southwestern Eye Center offices to be focused on taking care of you and your vision and eye care needs — more time spent interacting with our staff and our doctors and less time spent in front of paperwork and discussing insurance. Here you’ll find everything you need whether it’s your first visit or you’re coming back to see us.
We’ve streamlined our patient form process with Mobile Pre-Check! This will allow us to spend more time on you, not your paperwork. As your appointment approaches, you will receive an email or text notification from Southwestern Eye Center to confirm your appointment and complete your registration paperwork prior to your visit. Please ensure that you provide an accurate phone number and email address when you schedule your appointment to ensure that we are able to get in contact with you.

Complete the Mobile Pre-Check process via your home computer, tablet, or smartphone and your check-in will be expedited on the day of your appointment.
